What is Botox?

Botox is the popular commercial name for botulinum toxin. Botulinum toxin is known for causing botulism.

Botulinum toxin is produced by the bacteria Clostridium botulinum. Clostridium botulinum are harmless on their own – but the spores they produce carry the botulinum toxin which causes botulism – a very rare, but serious paralysing illness which affects the muscles of our face and mouth before spreading to the rest of the body. The word ‘botulism’ is derived from Latin for Sausages – as botulism was firstly properly diagnosed as resulting from the consumption of putrid sausages.

Botulinum toxin is widely considered to be one of the most dangerous toxins on the planet – being far more deadly than potassium cyanide.

Clostridium botulinum comes in a variety of strain, known as serotypes. These are A, B, C1, C2, D, E, F, and G. Out of these, only the serotypes A and B are used to create the clinically used Botox.

If Botulinum toxin is so dangerous, how is Botox treatment so safe?

How does Botox Treatment Work?

Botulinum toxin when used in small doses by a professional is harmless and one of the most effective and simple there is.

Botulinum toxin is injected into the muscle where it spreads through the nerve and binds. This means that ‘acetylcholine’ – a neurotransmitter – cannot be released. As a result, muscle activity ceases. By careful application, Botox can be used to stop facial spasms and reduce wrinkles in the face – effectively paralyzing the wrinkles and causing them to disappear.

Botox treatment is also used in the treatment of headaches; this is because botulinum toxin blocks the release of pain mediators – allowing it to effectively combat migraines.

Botox can result in bruising, swelling, headaches, and temporary muscle weakness – but these rarely last long. Furthermore, the overall effects of Botox are temporary – as the initial chemical process wears off and nerve ends regain their functionality. Botox tends to work for round six months before nerves begin regenerating.

The Origins of Botulinum Toxin Use

Clostridium botulinum toxin was discovered in the late 1800s, and by the 1940s it was properly isolated in a crystalline form. It did not find a medical use until the 1970s.

Here, Botulinum Toxin was used to help treat crossed eyes – strabismus (crossed eyes) and constant eye twitching (blepharospasm). At the time, these conditions required extensive, demanding surgery to treat and often repeat surgery was necessary. Much of this research was the work of Dr Alan. B Scott. He found that small injections relaxed muscles with no negative side effects. A significant number of studies and papers were released in the 1980s advocating for the use of botulinum toxin – not only in ophthalmology, but also for neck, face, and shoulder spasms.

The company Allergan licensed the treatment of Botulinum Toxin in 1989 after receiving FA approval for therapeutic use, branding it as ‘Botox’.

Since then, Botox has found a wide variety of uses. These include,

  • Cervical dystonia (approved in 2000)
  • Chronic migraines and upper lip spasticity (approved in 2010)
  • Excessive sweating (axillary hyperhidrosis) (approved in 2004)
  • Urinary incontinence (approved in 2011)

The cosmetic value of Botox was discovered largely by accident by Dr Jean Carruthers, an ophthalmologist in Canada. She found that her blepharospasm lost the wrinkles in their foreheads when provided with Botox treatments. She and her husband, Alastair Carruthers, went onto publish a paper in 1992 on the viability of Botox as a treatment for facial wrinkles.
